What is the physiological reason swelling takes place?

1. Often after the surgical removal of a breast, lymph nodes, and lymphatic vessels—especially those in the upper arm—swelling occurs. What is the physiological reason swelling takes place?

2. Some surgical treatments for breast cancer involve removal of lymph nodes in the axillary region. Why might this have a therapeutic effect in some cases?

3. How does the circulation of lymph differ from that of blood?

1. It is caused by interruption of the lymph drainage in the armpit (axilla), which can cause discomfort and swelling and an abnormal appearance of the arm. Removing lymph nodes and vessels from the underarm changes the way the lymph fluid flows within that side of the upper body. This makes it more difficult for fluid in the arm to circulate to other parts of the body. If the remaining lymph vessels cannot remove enough of the fluid in the breast and underarm area through diffusion, the excess fluid builds up and causes swelling, or lymphedema.

2. During surgery for breast cancer, the doctor usually removes at least one of the lymph nodes from the underarm area to see if the cancer has spread. Some lymph vessels that carry fluid from the arm to the rest of the body are also removed because they are intertwined with the nodes.

3. Just as blood circulates throughout one’s body so does the lymph. These lymphatic vessels flow alongside one’s blood vessels. However, unlike blood, lymph does not flow because of a heart “pump”. Lymph flows slowly, coaxed by the movement of the body’s muscles, until it empties back into the blood vessels just above the heart. Lymph only moves in one direction, toward the heart.
